As malasuerte says, when the standard says 'shall have a documented process', it means you need to have a process, and a document which describes that process.
My personal recommendation is to use a process flowchart, as this makes it easier to identify the inputs and outputs, show links to other processes (where necessary) and identify stakeholders/contributors to the process.
Nothing in the standard says HOW you have to document the process, but be prepared to justify your rationale to an auditor if you do something unusual.
