Increasing PFMEA occurrence ranking after non-conformance

We recently had a few claims for an issue and I think that should affect the occurrence ranking. That is, I don't think our previously stated ranking is accurate anymore since we had a few cases. Even though we have implemented a corrective action that will help detection, I am wondering if I should make adjustments to the occurrence ranking. Any feedback would be appreciated.

It depends. If your initial rating was based on insufficient data, or if the occurrence has increased since it was done, I think you are justified in revising the rating. However, if the recent claims are still in line with the original rating, I would leave it as is.

Are your documents revision controlled?
According to the practice, you may change the occurrence rating now (revision control-reason to update document), the second part is after the action items are updated/ completed, you may update the new occurrence rating after validation. Some time you may validate using x number of pieces or x number of months of production.

You shall review, so you have reviewed.
You shall update when necessary, but what does it mean?
E.g. you put in PFMEA rank 1 as per criteria described on page 93 of PFMEA manual and now your customer detected 1 nonconforming part.
Updating is necessary and you can't leave 1 there.
Count how much parts you've made since moment when rank 1 was given and then you will get new rank value. 2, 3, ...
Same applies - you gave 2, re-calculated and then maybe 2 still would be adequate, or alternatively 3, 4, ...

Thanks for the great feedback everyone. Yes - our document is revision controlled. I like your suggestion as I am not so confident our current countermeasure is very strong.
