It's very similar to the database I'm building for the oil producer.
The database is for a quality inspection on flowline, the gauges etc. They want a standard inspection form with options to add additional items. The additional items are dependant upon the location and type of flow line, cvr, steam, oil. The database will produce their forms, and it will store quality audit data and produce reports.

What verson of MS Access do you crrently use?
(FYI, I was originally looking at this board to find some information re: SOP and work instruction standards. I build databases...my bread and butter job, but, I am also in charge of a continuous improvement department in the oil service industry. We are embarking on a journey of ISO Quality System documentation and document control. We are also developing and implementing a system for process audits...my database building sure comes in handy.) Any thoughts? Experience in SOP/work instruction documents?
