Exactly. This is my main concern - inspecting the quality into the parts is the most difficult thing and this is where one has to be dependent on visual or judgmental mode of conclusion and the deficiencies are detected only when the 'part' suffers a premature failure.

Our tests were designed to assess the processed materials' ability to exhibit the ability to perform in the way it is designed to do. We duplicated the stresses and exaggerated them to see at what point the material would fail - fibers pull apart, adhered surfaces separate, combustibles burn, a sharp object penetrates fully.

We didn't test the raw goods, we tested fabricated samples after the processes rendered them in the state of customers' use.

Our product is Cement, Ordinary Portland Cement (OPC) & Portland Pozzolana Cement (PPC). We have to do extensive testings of all the raw material, intermediate & finished products at each stage of manufacturing because we can't verify all the specified parameters by subsequent monitoring or measurement before the product is shipped or even before it's used by the end users and as such we need to validate the manufacturing process. Testing starts even before the principle raw material (limestone in this case) is mined out; i.e., when the blast holes are drilled.

Also, ISO 29582-1 deals with cement testing.

ASTM also has standards for testing cement of various types.

Thanks Jennifer for the link to 'Concrete Testing Manual' which can be a useful guide for our marketing guys who has to deal with onsite concrete related issues while dealing with the end users.

ASTM specifications are applicable in US and some South/Middle East Asian countries. In India, cement, in general, is produced conforming to IS specifications. However, if it's to be exported to the above said countries, it has to conform to ASTM/ BS12/ Euro standards.

What I want to convey is that it's always not possible to inspect and verify all the critical parameters unless the material is put to use and it's where the deficiencies become apparent which couldn't be detected during the physical inspection at the warehouse (prior to issue and use). By this time, the documents have already been cleared and payment released to the suppliers.

For example, the stores receive a supply of nut bolts requested by me. Now I'm asked to inspect the material. Quantity, make, dimensions, packaging etc. are OK and as such I have to sign off the 'Inspection Sheet' and accept the material since it meets the 'prescribed specifications'. Well, I use this material and after 2 weeks I happen to discover that the bolts have suffered a premature failure resulting in production loss for 4 hours. (Although the suppliers later on made a free replacement but the loss incurred by the company was many times higher than the original cost of the faulty material).

The reason (root cause) for the failure was poor quality/ craftsmanship which couldn't be inspected and verified by physical means. What one can and should do to prevent such failures.

Has anyone experienced similar problems? Please share how you succeeded in managing the issues.
