Management would like to obtain satisfaction levels of departments within the company. A survey will be handed out to company personnel to rate the service of the another department. This "survey" will only be a handful of generic "service" based questions and the rating scale will be 1 through 5 (values of 1 -5 yet to be determined). This survey is to be applicable to any department in the company. So, it has to be very generic and only service-oriented.
Anyone have positive or negative feedback on doing something like this?
Have any examples that I could use to help me create my own?
