Internal Auditing Reports / Documents - Design and Content

Hello everyone

I have just started a new job role as Operational Audit Manager with my company. I have been with the same company for 17 years and just moved across from Contract Managemt. My first task in my new role is to re-design out Internal Auding report(currently 14 pages!!) Can any one help with any document styles, layouts,what should and souldn't be included. The current document has been produced in word and is approx 10 yeras old. I need to bring the up to date, user friendly, photos, and be two way between the auditor and the service centre manager whos centre is being audited. Any ideas, help ???? I have searched the internet but found nothing.
