"Lessons Learned System" - Evidence for Audit

Hi. Today's my first day at work ... at my old office BTW, after 3.5 yrs. Long story ... but quite frankly ... a feel little bit rusty

We are having a Supplier Assessment, they wrote: "No visual tracking of issues or evidence of layered. No lessons learned system in place"

For the visual tracking issues, I thought that having Quality Alerts showings the problem found and it's correction was enough, I'll have to think about how to satisfy this observation.

For the "lessons learned system", can somebody give an idea of what this means?

probably the meaning could be: Is there in place a process for problem in solving in the organization? Are some records available? How does the organization learn from their problems?....
I do not know the evidences you showed to the auditor, so you should be more specific.
I assume he expected to see some records ( and so a "tool") and indirectly understand and evaluate if there is process in place.
Overall this is an aspect of the continuous improvement process ( check for this clause 8.5.1 of ISO 9001 ans successive if you are involved in ISO TS certifcation).

Hi !

You can install an intranet Lessons Learned tool with a responsible to overview and manage it. Or, in case of an audit that sill come sooner, you can thin specificly per each project and look for the closed actions - from these ones you can select the onse that can be considered lessons learned.

Where does ISO/TS actually require Lessons Learned? I thought it was in CAR's or APQP, but I'm not finding it in the most recent version.

This is an old thread from 2010.

As far as I know there is no requirement in TS/ISO for a "Lessons Learned" however, a lot of companies use these as a continual improvement process. Just my opinion.

Appears to be a customer requirement, not a TS requirement. The customer is evaluating the original poster's company.

I think the question here is not how an organisation collects and lists lessons learned but what does the organisation now do differenly that is attributable to a lesson learned. This is a much harder thing to show evidence of and to trace.

Unfortunately I myself am struggleing with this to so I cant add any help.

Language translation can sometimes create havoc with terms used in various ISO standards. With a bit of stretching, I can relate lessons learned to CA reports that some organizations used - where the CARs were listed and categorized. Lessons learned from these past incidents become a valuable resource or input for preventive actions.
