I'm looking for a table that quantifies the probability of occurrence (in percentage / ppm) of a hazard in terms of remote, rare, negligible, etc. Does anyone out there has such table or guidance on matter.

You'll need to provide more information regarding what you're trying to do. The probability of occurrence of any given phenomenon is not necessarily related to the probablility of occurrence of any other, so you need to be more specific. Note that "remote," "rare" and "negligible" are all relative terms which can't defined without knowledge of the application.

The terminology used in the medical device industry when a conducting health hazard evaluation in relation to the likelihood of a hazardous event occurring with a medical device is Remote, Rare, Occasional, Frequent, Continuous or variants of such. What I looking for is a matrix that expresses/equates these terms in mathematically form.
