Looking for a universal(?) example to illustrate what happens when an error occurs

I realize that every company will be unique and that there will not be a "one size fits all" example. I am trying to create an example for our use to show what happens in a company when an error takes place.

For example, the salesperson writes the order down wrong. Now it gets entered incorrectly. The cutting department might cut it wrong as a result but it was cut to the salesperson's dimensions, which were, of course, wrong. And so on and so forth.

I'd really like to start with an example NOT from our company so that no one is sitting there trying to figure out "did I do that?" or "That must have been Joe Schmo that did that" or whatever. An example of the "error flow" in another company/industry would be fantastic to illustrate here was THEIR error, here's how they document it, here's who takes care of it, etc etc

Is that clear? Please help! Thanks!

Another thing to illustrate is the cost associated with the error at each step.

I agree

We're just trying to show WHAT happens. We have a Process Failure Form here and employees are trained on how to fill it out. That's basically all they know - error happens, I fill out form as trained, I give it to my supervisor.

That's it - their knowledge of what happens and "where does it go?" ends right there. I want to be able to walk them through a ficticious (sp?) - fake! example and show how it all works.

Reminds me of an old saying I heard once. It is not particularly germane to a modern corporation, but it gets the idea across.


For want of a nail, the shoe was lost.
For want of a shoe, the horse was lost.
For want of a horse, the rider was lost.

And all for the want of a horseshoe nail.

A flowcharted ficticious or fake process, may be an ideal format for communicating the consequences of an error that is allowed to get past all of your process constraints.
The before and after scenario, would be presented very well using this format.
