I am not an expert on good management review, or much else. However, I wanted to reply to the question you posted.
ISO 9001:2000 identifies several inputs to management review. Most of these are in the form of "the results of these actions shall provide input to management review."
If you have a list of these required inputs, you are half way there. There are many templates and examples posted throughout the forum threads and at least one in

It is notable that a management review meeting should raise one important question: is the quality management system effective? This question is what management review should revolve around. Instead of looking at specific corrective actions, for example, you want to look at the total number of corrective actions, response time for the different builders of the corrective action as it goes through the company, etc.
Look at assessing the effectiveness of the quality management system as the goal behind management review. Might even include this as a check-box at the end of whatever forms you use, objective evidence and all.
