Do forms that apply to procedures need to be listed under the associated documents on the procedures?

Normally yes specially if this is work instruction. If I am a person using that document and I want to know what forms I need to fill out then yes, it's good to have it known. However, every company is different and some companies do not reference anything as it's all written down in one document (e.g. a traveler that points to all procedures, forms, materials, etc.). So the real answer is it depends. If it's a stand-alone procedure then it's best to point the user to the forms they need to use whether in the reference section or in the body of the procedure.

The forms are referenced throughout the procedure. However, at the begining of our procedures we have a section that list associated documents. I was wondering if i should be listing the form numbers in this section of the procedure as well as throughout the procedure as they apply.

If that is the format you use, then yes - you should probably do it that way.

Usually, just listing the reference documents throughout the procedure gets the job done in my book, but I would suggest (if you haven't done so already) that you re-visit your control of documents procedure. It should specify how this is accomplished.

If you're referring to ISO 9001, there is no requirement for a procedure to reference associated documents or forms. If your own internal requirements cover that, it's a different story.

As a suggestion to answer to your question is to evaluate how effective you are in accomplishing your job in the case you do not have the forms enclosed in harmonic way in your procedure.
