That is why my NCM report has a spot to check if ca is required.
I agree Carol (sidney aslo) we complete our NCR with the disposition and then if the manager deems it necessary he raises a CA to investigate deeper into the cause of the issue (or several that seem linked)

No where in standards tells you how specifically you should setup the system, all they want is that you comply to it. Best process put for this is to have everything in one form if possible. Nonconformity, disposition, correctinve action. Sometimes the Car is by your company and sometimes by outside source(vendor, supplier). As long as you state what you are doing with this form and what you're using for in your companies procedure explaining the outcome of your process , than you will comply with the standard. Remember the standard is there just to guide you and not to set rules on how to perform it, you just need to comply. Good luck.

I may be a little late to the conversation but what I'm looking for is a standard generic NCR form I can use that can also include the MRB actions to be taken...an all-in-one...any ideas on where I can look?

We use two forms at my company. I had tried to incorporate one form, but my manager does not like the idea, how ever, the MRB is in fact very different than an the NCR. We also used to have three forms, a Nonconformance Report, A Corrective Action Report and the MRB form. When I came on board I believed that that was silly, I am a retired Airforce member, and was a flying Tech crewman and had to rely on taking as little as possbile, but the small amount I could store had to do as much as possible and repair as much as possible, otherwise I was in trouble when I was somewhere on the other side of the world. I blended the NCR and CAR report, and it became the our NCAR report, with an MRB block, my boss found that too radical so we have an NCAR report and the MRB form. I have now been able to slide an RCA block into the form with out much protesting, and I am now trying to introduce another major change to the form. I was successful in adding xreference blocks into the NCAR, where I could record my sources of investigative information. IE: Engineering Reports, ECO's, Audit Reports, Test records, Customer complaints etc. I did this as is speeds up time, this way on one form, I have documented where I am pulling my information from wrt to the NCAR. My biggest hurdle at the moment is trying to teach my boss the difference between Corrective action and preventative action.

This is my first response, sorry for the bable, but if you can do it with the least amount of forms and reports but be able to keep it simple but at the same time collect the most information then the form will be used more readily.

In our system we use one form for product non-conformances. Just one NCR form. I don't understand why one would need an MRB form. The Material Review Board is just a group of people, and the results of that meeting should be the Corrective Action. Our NCR form includes the problem description, MRB team, disposition (and temporary remedial instructions), root cause, and corrective action. The problem description can be filled out by anyone, and given to their manager. The disposition can be signed off before the Corrective action is finished in order to speed up the process.

I am, however, thinking of splitting it up into two forms (NCR and CAR). The NCR would only have problem description and disposition. If corrective action was required, it would move on to the CA process. We also have to capture audit non-conformances and process non-conformances. Currently the CAR form is used for this, but if I split the NCR form up, I'll split the CAR form as well, so two forms for capturing problems (product non conformances and process non conformances) will segue into one CA form.

Food for thought:
"Sometimes," the MRB determines the original NCR was not valid. Usually, a good MRB report details the root cause investigation,while most NCR do not. Further, MRBs are usually empowered to work out "compromises" having to do with rebates, refunds, discounts, use-as-is, etc. regardless whether the MRB organization is the aggrieved party [customer] or supplier. The last part is one of the main advantages of having cross-functional MRB teams.

Be a little carefull! In some sectors (including Aerospace and Defense) MRB indicates the authority to internally authorize the shipment of non-conforming material to the customer without authorization. The term MRB has a different meaning. Make sure you know what your customers think MRB means. I have several customers that do not delegate MRB authority to our organization.
