We use two forms at my company. I had tried to incorporate one form, but my manager does not like the idea, how ever, the MRB is in fact very different than an the NCR. We also used to have three forms, a Nonconformance Report, A Corrective Action Report and the MRB form. When I came on board I believed that that was silly, I am a retired Airforce member, and was a flying Tech crewman and had to rely on taking as little as possbile, but the small amount I could store had to do as much as possible and repair as much as possible, otherwise I was in trouble when I was somewhere on the other side of the world. I blended the NCR and CAR report, and it became the our NCAR report, with an MRB block, my boss found that too radical so we have an NCAR report and the MRB form. I have now been able to slide an RCA block into the form with out much protesting, and I am now trying to introduce another major change to the form. I was successful in adding xreference blocks into the NCAR, where I could record my sources of investigative information. IE: Engineering Reports, ECO's, Audit Reports, Test records, Customer complaints etc. I did this as is speeds up time, this way on one form, I have documented where I am pulling my information from wrt to the NCAR. My biggest hurdle at the moment is trying to teach my boss the difference between Corrective action and preventative action.
This is my first response, sorry for the bable, but if you can do it with the least amount of forms and reports but be able to keep it simple but at the same time collect the most information then the form will be used more readily.
