Try never to say CAPA because PACA may impart better understanding of preventive action.

As with so many things context matters. It is the entire phrase that conveys the intention. The single word does not. Focusing on a single word is a red herring to discussing the intent of different actions.

A simple but effective approach I've used for managing many 8Ds and ultimatley their close out....the PM/or their designee maintains a spreasheet of Internal or External (customer facing) Open Issues....given that an 8D usually involves some form of customer input, the 8D and its effectiveness is updated during weekly or bi-weekly Open Issues meeting usuallly on line...if in the automotive arena the 8D progress/effectiveness can also be addressed in the PA process audit spreadsheet
