In simple terms, I would explain these slightly differently.
Performance is about measuring "something".
Example 1: We made 3 widgets today. Our performance was 3!
Example 2: Our employee turnover for the last year was 12.
Effectiveness is then using that measurement of performance and comparing it to a "standard"
Example 1: We made 3 widgets today. Our performance was 3! Our expectation or goal was 4. We are not 100% effective.
Example 2: Our employee turnover for the last year was 12. Our goal was to have less than 15 employees turnover. We were effective in meeting our goal.
I would say that the requirement in 9.1.1 is as much about the measurement of the performance as much as it is about determining the effectiveness of the system.
