PFMEA - How long should the recommended actions remain in the recommended actions column?

#1
How long should the recommended actions remain in the recommended actions column? The person who updates our PFMEA's doesn't ever want to remove them. So there are actions listed from 2 years ago. We are doing an annual validation PPAP and he's updated the PFMEA but he wants to keep it there. Shouldn't that now be added to the control detection or prevention columns along with the new severity, occurrence or detection ranking numbers, whichever may apply? After 2 years those actions have now been fully implemented and are effective.

If the recommended actions from two years ago have been completed, then this should be reflected in your updated FMEA. This revised FMEA would then become the next living reincarnation.

You asked how long. I agree with UF. Once actions are implemented, validated and deemed effective, they are no longer recommended actions. When the PFMEA is updated (annually, apparently, in your system), completed improvements become part of the current system.

If the other person wants a way to show how much risk reduction was achieved as the result of improvements the past 2 years, compare the RPN score of the current PFMEA to previous revisions. I don't want to speculate on the other person's logic in not updating the current RPN score. Can you elaborate on his reasoning?
