PFMEA (Process FMEA) Class Symbols

Hi All,

Im currently working with a supplier and I need the standard symbol if there is one for Safety critical. It been a while since I had FMEA that if a certian failure mode occurs it could harm the operator without warning. The ranking of course is a 10. But, I need a symbol to standout in the Class column. Any help would be appreiciated.

There is no "standard" symbol. Note that the "10" should be enough to alert interested parties that something important is going on, but you're free to create your own symbol or co-opt one from somewhere else.

Other way of communicating sevearity:
Fix-up cut-off RPN (When action is initiated)
Highlite RPN rankings -
less than cut-off in green,
More than cut-off in Red.
Red alarms for CA

It's not generally considered to be a good idea to assign "cut-off" or trigger values for RPNs (i.e., an RPN > x requires action). There are two reasons for this: when there is a threshold limit, RPN values tend to cluster below it (as a way of avoiding extra work) and the strategy should be to evaluate each FMEA line item for opportunities, regardless of RPN (although those with high severity numbers should get attention first).

While making a FMEAS u have to choose a symbol for critical, safety as well as major & minor.

Its not a standard symbol for those, U decide the symbol for each of one.

The symbols are customer specific. For example, if the part is to be supplied to Ford, the symbol is the inverted delta. If the customer does not specify a symbol, or if the product goes to multiple customers, you can use an internal symbol that you can cross-reference to customer-specific symbols.

As Jim pointed out, there are no standard symbols. Safety Critical usually applies if the Severity rating is a 9 or 10, if you are using the standard rating scale. However, this may change if you have customized the scale.

There's no standard requirement for symbols, although many customers require them. If there's no stated or perceived need for symbols, the "Class" column may be left blank.
