Sorry but I don't think the rating for failure mode will be an average of rating of causes. It is but natural but if we gives rating first to failure mode; occurance ranking for failure mode will be always greater than the ranking of causes.

Typically (in a Process FMEA) Occurrence of Cause and Detection of Failure Mode.

The Occurrence of the Cause is however often not a readily available metric. Ocuurance is however NOT just from history. It will take into consideration the Preventive Measures planned and in the absence of historical Data, the experience of the FMEA team.

The metric for Detection is normally fairly sound as it relies on the detection activity planned.

In both cases it is critical that the Tables are designed to-suit the organizations specific. Typical AIAG - scales are JUST examples. Occurrence depends on an organisations volumes and detection depends on the type of product/process.

So many people make the error of using the EXAMPLE scales, without modification - and worse - there are still Auditors out there who state that an organization MUST use AIAG scales:notme:

AIAG mannual is just an example. We can refer that for ranking.
But as per my openion along with my company FMEA team, occurance ranking for failure mode will be more than the occurance ranking of individual causes for the concern failure mode.
Occurence ranking for failure mode will not be an averge of occurence ranking of causes.

Am I right or wrong?

Wrong!
Failure Causes will appear in SEPARATE rows and therefore can be ranked individually.

The Prevention actions will/may also be different for each Cause.

Grouping is possible on Prevention Activities - against each cause- ie where we have multiple prevention for One Cause, we take an average.

Same applies for Detection where multiple Detection exists for one Failure Mode

But I'm saying that the ranking for failure mode will not be equal to the average of the rankings given to its causes.

Not seperate Columns! Each Cause will be in a separate ROW and therefore each cause will / may have a different ranking & different prevention measures therefore there is no averaging out.

ie. one failure mode may have several causes and each cause is addressed separately in a separate row. (underneath each other with a common Failure Mode)
