PFMEA RPN - Feedback regarding adjusted RPN

Hello - Looking for some help with a problem we recently had. We shipped a non-standard part (missing a component) to the customer and now need to include this failure mode on the PFMEA. Please review our ranking numbers and provide feedback regarding adjusted RPN (still very high at 216). Are we being overly critical/cautious? View attachment fmea.pdf

What are the scales used for the different rating categories?

You have a 9 for Severity. This is typically used to indicate a failure mode that could cause bodily harm or property damage.

Thanks for responding -

We supply emission control devices which have federal regulations so almost all our severities are rated at a 9. We use the AIAG FMEA handbook at our guide.

Without knowing the details of your operation, the numbers seem reasonable.
