Isn't each Poka Yoke an prevention based on detection?
Where can I find the real definition of poka yoke?

I recommend the book "Poka Yoke" by Shigeo Shingo. It contains many examples with sketches and photos.
This link also gives a concise work instruction on the topic.

Section 4b discusses prevention-type Mistake Proofing, and section 5 discusses the detection-type. Process-design Poka Yoke is typically called Mistake Proofing. This can be either prevention or detection.
Component-design Poka Yoke is usually called Error Proofing. This is designing a component such that you cannot assemble it incorrectly (e.g., part is symmetrical such that there is no backwards, upside-down etc., or part has a unique feature that prevents incorrect assembly). This is true prevention.
