In the version I have, this is referred to as 'Traditional problem solving' which I think, given the frequency with which it happens, is absolutely the right term for it.
A serious point for this one. Trying to understand the reasoning in this chart has helped me to change the thinking towards problem solving by bringing into the open some of the causes why the problem solving that I want doesn't come about. So whilst it is a very good representation (and funny as well, I have an Australian version by the way which contains some colourful expressions!) I would urge anyone to try to understand why this happens in their organisation, you may be surprised at the results, and may even uncover some of the real root causes of the intermittent operation of the problem solving process.

The real problem is that even when someone has seen it and the brown stuff momentarily hits the fan, that's what it is, it is momentarily. Everyone continues afterwards pretending business as usual.
Management is aware of this but lacks the balls (excuse me, ladies) to take a firm stand.
But hey, it causes good laughs during lunch break, and that's all we need.
