So, your QMS is separated from your organization's business system which determines how opportunities are fulfilled while managing the risks?
Better to merge the two to reflect the one system that is used and improved to determine and fulfill customer needs.
You could make sure that the upkeep of every process and its related documentation is the responsibility of the person who knows how the process works from end to end. Top management selecting the best people as process owners may also address your concerns about monitoring the usage and continual improvement of your system, its processes and its SOPs.
