QMS Update - Rev. up or start over?

I am currently in the process of updating our quality manual. There have been zero updates since the company was ISO certified several years ago (prior to the oilfield tanking).
We are now in the (very) beginning stages of working on AS9100 certification.
My question is, if I completely revamp the QMS should I still rev. up to revision B or start over as a new QMS at revision A?

Other than a few minor changes/differences what's wrong with what you've got?

Keep things small and expect small problems, do things huge and watch out.

You can update the issue no. and make the revision no. "0". Issue no. can be used whenever you are making major changes to documents / it is difficult to track revisions done in a document.
For new document, issue no. is 1 and revision no. is 0. If this was a new document, it might be having Issue no. "1", you can simply give it new issure no. i.e. "2" and Rev. No. "0".

When we aded AS9100 to our company we run two QMS systems in tandem due to a split between Commercial and aerospace work in the company so we just cut and pasted over and added little bits like Randy said. Biggest issue we mainly had was questioning whether it actually was that easy.

Combining requirements for more than one standard can be challenging, depending on the standards involved. What is your primary business? Do your Quality Manual according to that, and add in the bits for the second standard.

As a Doc Control manager, I would say make it whatever the next revision level is according to your document management system. I am assuming you have that defined already.

FYI -

We have been using a paper based QMS with lettering for referencing our revisions. We now are releasing an eQMS and will be switching to revision numbers. The change, letters to numbers, will be identified in our document control procedure.
