I've seen the Administrative Assistant (Secretary) to the CEO designated the QMR and to be honest the supervisor of janitorial services could be a QMR.
The organization defines who is a manager in the orgaization and it doesn't have anything to do with what the person is called. The QMR can be the manager of the quality system and nothing else and not have one person directly under them. (Don't get this confused with a Quality Manager...apples & oranges)
