Quality Management System (QMS) 4.1, a and b

What is the best way to satisfy 4.1, a and b in the new standard? Right now we use a matrix which defines who has primary and secondary responsibilities for each element. We also list all the numbers of all the documents for each element. Could this be expanded upon to cover this?

RE: 4.1

I created a very simple flow chart in my quality manual for the interaction between the processes of the qms. This covers 4.1 (B) and also 4.2.2 (C). It is a very generic chart that just gives and overview of the entire system. I reference the applicable operational procedure for the gritty details.
