We are a contract manufacturer for a manufacturer of an export use only medical device. Are we required to register and list as the contract manufacturer in this scenario? I could not find a way to do this on FDA registration site. When I selected the box for export use only, it didn't give me the option to indicate that I was the contract manufacturer. The manufacturer has correctly registered and listed the device for export use only.
