To deal with unreliable suppliers, companies need to have multiple suppliers for every item
In the past, companies dealt with this through vertical integration (everything in-house)

I would have to agree with, but the real person who should be punish due to the poor quality is their QA personnels.

IMHO, blaming a supplier for quality issue is only a short term solution.
Suppliers should be viewed as part of the family. As a legal manufacturer, the company will be held accountable for the finished product.
