Renaming CARs so that they do not seem negative

On the renaming aspect, it seems to me that if you identify that cute little cat sized animal that is mostly black with two white stripes running down its back as a skunk, that isn't going to keep people from thinking any differently about it if you call it rose.

A skunk by any other name still smells as foul.

I agree. But bosses have suggested changing the name.

With all agreement about the fact that name change does not change the case on hand, the aspect I see here is not the case on hand, but the motivation to handle the case.
An other way to communicate something is wrong perhaps is by telling "could be better" where the word wrong is removed and better thoughts and ideas emerge as one begins to think how anything could be made better ...

When management starts rewarding people for identifying and solving PROBLEMS instead of behaving like they are a bad thing it won't matter what you call them.

The folks who avoid your CPARs like the plague do so because your management has created the situation; only they can change it.

There is also the component of what's contained in the CAR. Frequently, the issue os of little significance to management, often reported in terms of the ISO standard and they don't understand, but are expected to be all over it like a rash, to fix it.

Until we, especially as internal auditors, start reporting things worthy of fixing, then it's true, renaming it won't make a difference. I'd vote for doing both, since a CAR is purely an overhang from supplier audits, which many organizations have zero experience of dealing with.

Two more excellent reasons why being taught to audit, based on external principles is wrong and out of date!
