Don't bother revising it. Delete it completely. This type of checklist, based on turning the standard into a bunch of questions is not what audits are about. I'm glad you see the need to do something different, but I believe it's not the form, it's only part. It's the way you go about auditing that needs to change too.
Did you have any training on auditing? Did the trainer tell you to use such an approach? If so, I'm afraid you were robbed! The checklist doesn't support the 'process approach' and you will have to change a lot more to be able to develop a more effective set of audit techniques.
May suggest some help? Either to look over the whole internal audit system you have and see what's required to move it to a process methodology or to encourage you to get process based auditor training...
