Does the date in your revision history correspond to the date the the changes were made in the document or the date that you released it?

Typically when it becomes effective - so when it's released! But I'm not sure if that is something defined by any particular standard. Maybe just a rule of thumb in the industry in general? I could be wrong! :geek:

The important thing to know / convey is when the update is effective. How you indicate / communicate that is up to you (and, of course, should be consistent). As @blackholequasar notes, it's pretty typical to communicate that in the revision history but there's no standards / regulatory defined mechanism. I've seen cases where the last approval date is considered the effective date and the revision history has no dates at all.
