Do you need a Revision History Page on a Form? I've seen that done, and it seems a waste of paper if you have a one page form, and then the second page is the Revision History Page of that form.
If you use a document control software, there is a information card for that document that tells you what doc #, rev and title it is. There are usually other fields that describe changes made.
Am I correct in assuming this?
