Revision Levels on Documents - First Issue Revision Level

I'm sure this has been covered by someone, but a search didn't give me a definitive answer. On internal documents, is the first issue revision level "revision: None", "revision: -" , or "revision A"? This has never come up in an audit before, but this time our auditor questioned my use of "revision: None". I stated that it had not been revised, therefore there is NO revision. A couple of days later I was audited by a customer and I noticed that a couple of his documents were "revision: None".
We are a job shop with 400 active customers so I can't control their methods but I would like to be sure that my companies are correct.

I see no problem in the "first" emission of the document being Revision : none, 0, zero, A, 1 it´s all up to you....

If your auditor questioned the use of Revision : None out of curiosity is one thing, but if he seriously questioned it, then I´d be worried... didn´t he have anything BIGGER and more important to be questioning ? :notme:

Ted, He must not have had anything else to worry about because this is the first audit in 8 years that we had zero finidngs. Not 1 CAR to write. He did give me a few opportunities for improvement, but that was it.

You should have documentation that spells out your requirements for revision numbering; if it says to use "none" for the first issue, then I don't see what the problem might be. Even at that, if everyone understands what "none" means, it shouldn't make any difference. You could use just about anything, and as long as the meaning is clear to those who need to know, it's a non-issue.

This is a silly question, but if you do a revision history- do you need to include in the history the current revisions you are making to the document? I hope someone can help me with this. Thank you!

If you decide to maintain a revision history, it typically includes the changes that were made to the current version. That way, the users can see what was changed in the current version. This is very common (for example) with engineering drawings.

How you handle the control of the revision history is up to your organization.

I will admit that my organization uses the revision history to assist in training on a revised (and approved) document. Rather than re-reading the document and asking "What's changed?", a User can simply scroll to the revision history and see the changes. This allows for a more focused training session.

That being said, I know that some organizations keep their revision history separate as they believe that it's too lengthy and "clutters up" the document. If this is the case, as part of your document control process, it could be defined that any hard copies out on the floor are controlled and do not include the revision history which is maintained with the master hard copy.

It's all about making the system work for your organization.

….another approach is through document change notice (if you are using this type of form)… some organization used document change notice to review & approve proposed changes to the manual, procedures and forms (i.e. initiated by concerned personnel and reviewed & approved by MR)… and with this, they don’t need to have an history record of amendments made since this will be available on their DCN records…

Gee, he must really have been looking for something to raise!

But the rules can be whatever you make them.

I use the term 'Version' instead, and use system development terminology: so its v0.1, 0.2, 0.3 in draft until the first signed off issue, which is v1. Then v1.1, v1.2 etc.
