AndyN and Jennifer, You both make great points about the OFI! I do have documentation showing that I have atleast looked at each OFI and a couple I have implemented into our procedures, as they were actually good ideas.
I did not know that you could create and use a matrix without it being accompanied by a procedure. That should work for the HR portion. Our training is done in-house, and I have a master employee training list as well as individual training records in each employees files.

This is a very good discussion. :agree1: It's a great time to point out that ISO 9001:2008 doesn't specifically call out for a Planned Maintenance process. It does say (6.3, emphasis mine)
The organization shall determine, provide and maintain the infrastructure needed to achieve conformity to
product requirements. Infrastructure includes, as applicable,
a) buildings, workspace and associated utilities,
b) process equipment (both hardware and software), and
c) supporting services (such as transport, communication or information systems).
...but it's not calling out for a written procedure for any of these three - nor do the HR elements 6.2 and 6.2.2 specifically call out for a written procedure. The standard is asking for a process, which auditors can establish exists when reviewing evidence of their function (this is where a matrix can come in) and by talking with people who use the processes.
