Root Cause Analysis when someone simply stops performing a task?

Nobody should be doing RCA for the sake of doing RCA. RCA is just one step towards the real goal of preventing recurrence. If the "self audit" activity is really important, the monthly results should be mandatorily shared with his/her line manager. The problem would have been uncovered after 1 month and not 6 months later. On the other hand, this exercise may have shown that the self audits were totally useless, by the simple fact they were not performed/reported for half a year and nobody noticed.....:naughty:
Sidney not only is your assessment spot on....how or why did it take so long for this "hole in the process" to surface, likely indicative of other challenges....but I love the use of "mandatorily". Thanks Sidney...

This type of activity (monthly team self audit) is normally the result of a corrective action for a significant earlier problem. After all, nobody does monthly self audits for no reason.

Chances are, the lack of such self audits have deeper implications, but and once again, if the performance of such audits was “critical”, their performance and results should have been monitored more closely.

I would guess the reason was the same as for most routine QA audits, to assure someone of the quality of process outputs.

The original question indicates that the team was supposed to audit their work, but then says the manager stopped doing it, so it's not clear if the team was doing the auditing, or the manager. This makes it harder to guess who the audits might have been supposed to assure.

IF no one noticed it, does it need doing?
The audits might have been needed to maintain or improve the quality of the team's performance, but if no one noticed the auditing had stopped, I would guess no one was paying enough attention to notice whether this affected the quality of the team's performance, either.

I have a situation where I was auditing an internal team that was supposed to audit their work monthly. It seems that the manager responsible (no longer with the company) simply stopped doing it about 6 months before leaving. It's obviously an audit finding, but what's the root cause when someone simply stopped doing their job... and they're no longer available to explain why?

Thoughts?
My thoughts have lots of questions than answers:
  • Why are they auditing their own work?
  • Why is it on a monthly basis?
  • Who tasked them to do the monthly audit?
  • Why does it take 6 months to know that their not doing what they were tasked to do?
  • Why are you now auditing this team?
  • Who tasked you to audit this team who is supposed to audit their own work?
  • What value would you get to know the root cause of this specific situation?
