"Personnel allocated to investigating and writing up the accident"

When I mentioned this once the response was ‘well we have to pay you anyway so that's not a cost.”
But still it's a cost because the personnel would otherwise be engaged in some productive work even if the personnel is a fulltime safety officer. If not engaged in report writing, s/he would probably be thinking of preventing another potential accident during the time spent in investigation.

Someone said "we have no time for safety because whatever time we are left with is spent in reporting, investigation, corrective actions, analysis etc":D

That’s very good Jennifer. I like the way you do the FEMA's. I have rummaged through my safety stuff and I found these two attachments in case it helps anyone.

The idea is that a risk register is created and then the risk assessment matrix is ued to assess the risk in each hazard.

In the matrix, the severity of the hazard remains the same. E.g. if a vehicle crashes the impact or severity remains the same so only the likelihood and management of the event can change to reduce the risk.

I mostly agree with the above contents except for a minor point (2.1 of the attachment) where 'damage to property' has also been identified as a hazard which, according to OHSAS (2007) shouldn't be part of OHS risk assessment.

The definition of the term “hazard” no longer refers to “damage to property or damage to the workplace environment” (see 3.6).

Instead, the risk of such “damage” having an effect on occupational health and safety should be identified through the organization’s risk assessment process, and be controlled through the application of appropriate risk controls.
Now as to the question of severity (consequence once the incident takes place anyway) of the hazard which mostly (but not always) remains the same even after putting controls but there are cases where severity level is reduced moderately. For example, at hazard no. 2 in the attached sheet, controls like "availability of trained First Aiders, Location of first aid supplies – vehicles, Emergency Response Plan" etc. can add to minimize the severity. Their presence (and absence as well) does make a difference.

Consider the example of toddlers who normally are very bouncy & naughty (not sure if it's a correct term to use but anyway it's like this) and it's very difficult to control or reduce the likelihood of an incident resulting from their naughtiness. So we normally do something to reduce the severity of injury by providing additional things like cushions, carpets, mattresses etc. etc. around them so as to protect them from getting hurt.

I agree, there is a higher return on investment when time is spent preventing an occurrence than when investigating/analyzing/correcting one that does occur. "Proving" that to fatalistic, myopic people isn't easy, but a curve can be developed using the Safety Costs Calculator I attached in the Is safety in chemical industry an important requirement to be ISO certified? thread.

A person still needs to be prepared to deal with disappointment though. Personalities exist among us who, in the face of all data indicating logic in doing the pragmatic, sensible thing, will insist on doing things their way. Every now and then we learn about a spectacular fine or fatal accident that costs more than money: it costs public opinion, and unless one is a single source public opinion does, in fact impact sales.
