Seeking Sample quality system questionnaire which can be sent out to suppliers

Does anyone have a sample questionnaire which can be sent out to the vendors to be filled in so as to give a brief idea of their QMS and qualify them as Vendors to the company??

I think there's a supplier questionaire in the pdf_files directory. https://Elsmar.com/pdf_files

I think one is sup_eval.pdf and another is supplier-survey.pdf and another is Supplier_Audit_94.DOC
