From the information provided, it occurs to me there are a number of different factors to consider in this situation:
There is the issue of why staff are leaving, and over what period they left. It is important to understand the reasons for each so possibly not a single root cause or corrective action there.
There are then two systemic issues:
Resilience - having key positions or processes where there is no contingency leaves you open to problems with the loss of people, regardless of the reason.
Organizational knowledge - without the appropriate records or processes in place to allow for a job to be picked up by another person the resilience issue is amplified through not knowing what needs to be done, or picked up, in the event of a leaver.
