Should I send survey questionnaire prior to Supplier EMS Audit?

Hi Dr. L. Ramakrishnan, Would you mind if you can share additional points to include on a survey questionnaire like that? I mean, if you were in that position, what are those things you would ask about from your suppliers?

I have carried out supplier sustainability assessments till four years ago; now I don't have any survey questionnaire to share with you. However, I would like to suggest to you to visit: http://www.philips.nl/about/company/businesses/suppliers/suppliersustainability.page
which gives you a lot of downloadable information on supplier sustainability practices of the company where I worked. You may like to see the Supplier Sustainability Audit tool, especially the environmental section of it to formulate your own.

This is also an example of how MNCs address some of the environmental and social risks of outsourcing manufacturing activities to developing countries.
