Should the pFMEA start with Receiving Inspection?

I have read many threads on the pFMEA in the forum. However, I have a question:

Should the pFMEA start with receiving inspection?

As far as my limited knowledge is concerned, there is no actual processing done on the received parts and they are just inspected as is. So why should we start pFMEA with inward inspection based on the process flow chart? Am I missing something?

Please enlighten me on the topic. Thanks.

Actually I have started it with receiving as it arrives on the dock door. The firstg check is for overt physical damage. Then proceed with the remainder of the incoming process.

I've always thought that anything that can adversely affect product quality should be included. The thing to be aware of is to address only potential failures of the process itself--not stuff that your suppliers cause. You should be concerned with things like the likelihood of rejecting good material and accepting bad, damage caused by handling in receiving, improper marking or identification of material, etc.

I believe that a PFMEA should start at your first operation which could be moving product (indirect process) but not at Receiving Inspection. Risk and history should dictate where it begins. At one time, it was simply direct labour such as 1st Op, 2nd Op etc. but the newest AIAG standard has expanded that.

Your supplier should supply a PFMEA for their product and I hope that you can tell whether or not that the documents received were truly generated by a team or just paper work to satisfy a customer.

If, in your case 'p' = process, and your process starts with receiving materials, then why not? In the classical application, the pfmea is derived from the process from input to output...

I am sure David will respond. One way to find out is to talk with the Members of the FMEA Core Team. When I conduct audits, I want to speak with them. Interesting to see the reactions I get....;)

What you're looking for is evidence of conscientious analysis. There is no single set of clues or methods. Sometimes the number of potential failure modes for each operation is telling, or if the analysis seems simplistic and shallow.
