I was working on my Internal Audit Work Process SIPOC. Since I don't have a lot of internal resources to check if anything is missing. I thought I would put it out here for review. This is still a work in progress. I need to add the ATEX, EN13890, requirements for Quality Assurance & Management requirements.

I have seen the mapping that you have posted according to SIPOC diagram.
I was a bit confusing .
I think that the intent should be the mapping of the Internal audit (process).
Based upon the above method ( SIPOC stands for Supplier, Input, Process, Output, Customer), you shall indicate who are the Suppliers and what inputs give to the process that you want to map ( in this case Internal audit, in centre ) and who will be provided ( Customers) of the output of the mapped process.
In the mapping I think that "triggering point" ( what initiates the process) and "concluding event" ( what finishes the process) are missing as well as the intent of the process .

Hi,

The triggering point is the Management request {in form of audit plan as defined in the quality manual} or it can be customer request {agreement/contract}) and the concluding event is the audit report (with findings and OFI).

Please correct me if I am wrong.

Yes, but it also depends on the boundaries and the intent of the process to be mapped, that is usually described in a section called "Process description".
In a mapping for internal audit process, I would suggest "delegation of the audit authority" as a triggering event and "results of the planned internal audits communicated to management" as a concluding event .
These ones probably are wider than the ones you said but we are aligned.:bigwave:

Yes, I consider the main triggers from Top Management from the Quality Manual and the customer would be from a contract review. I will change that from customer request to reflect output from Contract review and a link to that process.

I was going to add the process intent just had not decided where on the form.

Other suggestion if you want.
In Input consider to insert the pre-process immediately before the process you are mapping and the related supplier. For example one could be: Management review and supplier could be Top Management. The processes that give the imput should be the ones you have put in the general QMS mapping as per 4.1 clause ( or related sub processes/activities). The same for the output whre you shall indicate the post process ( to the internal audit) and the customer as well as the output description.
As to metrics, consider to have at least one of efficiency and one for effectiveness.
Indicate clearly process owner, stewards as well as performes roles as well.

Are suppliers and customers the same?
Yes the supplier and customer are the usually the same for this process. The customer supplies the requested audit scope for the audit and the report goes to the customer.

Thanks for the question. I need to add the Audit Scope as the input.

Can a customer, external to the company, be in the C column for a CAPA SIPOC?

I would not as the "Customer" is the recipient of the "Process"

The way the SIPOC is built is that is -

Suppliers to the process : Contributors or who provide input to the process

Inputs provided by the supplier : What are the inputs provided to the process ?

Process : Where does it start and where does it end ?

Output of the process : What are the services or products offered to customers ?

Customers of the process : They are the consumders of this Process and often specifying their requirement.
