It is in the response from @Miner already, but wanted to add my take: You have a Root Cause for the Problem you have defined. It should be a 1 for 1 (note: you can have root cause types, different story).

The "subsequent issues" are not 'root causes'! They are separate issues and you should capture a root cause for each of those.

Yes, the subsecuent are not root causes, were detected problems for which I have to find the root cause as well, finally
having a solution with three root causes.

Yes it is and it was the first time you posted. Do you now have enough feedback to continue appropriately in determining the actionable ‘root causes’ of the maintenance problem, operator miss and inspector miss? Curious because you got the same responses this time as you did the first time…
