Can anyone please explain to me what is actually required to be TEST FAILURE RESOLUTION REPORT and its FORM.
Anyone before heard this report.?

Maintenance monitoring software - Test Failure Resolution Report

I guess johnjbart got lost, but I found this:

Brief: Maintenance software

Lockheed Martin Corp., Bethesda, Md., has developed the first version of maintenance monitoring software for the F-35 Joint Strike Fighter aircraft. The Test Failure Resolution Report software collects and stores failure data, root cause analysis data and corrective actions. As the first component of a Lockheed Martin F-35 distributed maintenance-tracking system, called the Autonomic Logistics Information System, the Web-based software is expected to reduce costs by making aircraft data easier to obtain by maintenance crews.

Sounds like a standard root cause corrective action, to me. I haven't found / seen any specific form, but in this case it would be a function of the software / database front end.
