Agreed, but ... Document the procedure IF the root cause analysis points to the lack of documentation and not in other direction. Don't jump the gun and just assume that the failure occured because you don't have a documented procedure. I have seen this done many times just for the problem to come up again at a later time.

Just curious to know how long a system must have been established to qualify for certification ? Here the tenure relates to 'maturity' of the system before it is deemed 'implemented'. Are there any guidelines for the CB's which must be taken into consideration before an EMS is granted certification status?
