Total Productive Maintenance (TPM) Portuguese Definitions and Terms

Hi all.

having a problem here. I usually do most of the TPM related training in our company. The thing is I do it in Portuguese. Now I am confronted with having to do it in English. So I am rewriting my documentation to English but am having problems finding some definitions and terms used. For instance, in Portuguese we have what is called "Cartão de Defeito" or "Cartão de Anomalia". Basiclly it's a card that the machine operator uses to signal, identify and communicate the existence of a defect in / on the equipment. I can't find the English equivalent. So far, the closest I have com is the Abnormality Assessment Form, but that's not it. Anyone know what I am talking about, what it's called and where I can find some relevant documentation pertaining to it?

It could be "quality deviation equipment form" or " quality deviation equipment note"

Thanks. My subsequent searches for these terms did not yield any results. So maybe it's not quite there yet. I did however find Defect Tag and Defect Tagging, but also cannot find documentation pertaining to either one. I am at the end of my rope and don't just want to do a literal translation and look like jerk that didn't do his research. :( Any other ideas? Here is a photo of our cards / tags / notes. Maybe a visual aid will jog some memories :) You'll have to copy / past the url. Sorry. But I still haven't hit the 10 post minimum to be able to post links.

I have seen the picture...You can use whatever name because they are self explicative. ANother option could be "malfunction/defect equiment check sheet":bigwave:
