Jim, sorry my question wasnt clear. I wanted to know if there is any requirement on the maximum RPN which could be mentioned for that FMEA plan ? Because I have seen FMEA's where most RPNs are below 100.

The individual factors are most likely relative and arbitrary, not absolute. Whether one assigns a value of 4 or 5 isn't dictated by any normative standard. I'm not sure what your point is regarding RPNs < 100. It's possible in this type of FMEA; it's just that the final number can't be more than 125 because the individual multiplicands can't be more than 5.

What would be the ideal RPN value, above which corrective actions have to be planned.

Typically, I have seen these values to be below 100 but in some documents have also ssen as high as 150 or 200.

No matter what type of FMEA is involved, there should never be a threshold or "trigger" value unless it's been prescribed by a customer. If the threshold is 100, you'll see a lot of values in the 90's. Each situation should be evaluated individually to determine whether or not improvement is possible and prudent. You might find that something with an RPN of 80 can be readily improved, while there isn't much that can be done about an RPN of 100 for another operation.

I think this is great, does anyone have a CP that they have completed that they would share. We just had a plant power outage and we really need something with this amount of detail.
