Revision: The act or process of revising...a revised or new version, as of a book or other written material (document).

Version: A particular form or variation of an earlier or original type.

Some context would be good, but in general, revision (editing, changing, rewriting, etc.) of a document results in a new version of it.
I should add that in some contexts, "revision" and "version" might mean the same thing. You might say that some document is revision 1 or version 1.
