Very Basic PFMEA (Process FMEA) Guidelines

We have a new group of individuals who will be working on our PFMEAs...and the organization has decided that even though they have no or very limited exposure to the process no training will be done...

So...I am looking for some very basic "rules" they can start with...

One of the problems with our current ones is items listed as failure modes are actually failures from earlier process in the operation.

So, I am trying to come up with an easy set of guidelines as to what should be included and not included (as a starting point). We have one PFMEA where every failure mode listed (25 of them) are concerning incoming material (from the previous process)...

I think this PFMEA light link is an excellent training tool, thanks for posting it. When creating PFMEAs from nothing, ya got to start somewhere - and that is with the process map. The point is, everyone started at the same place! "I think this is what our risks are...okay what needs to be controlled?" And then it grows. PFMEA is a living tool, it improves over time as additional failure modes are identified. That's why you review the historical defects as well as ensure every defect you every experience from the point forward is pulled up against the PFMEA, to see what you missed!

In emerging markets where we are just learning the value of inspection control charts, process control charts, and PFMEA, I think there is a real need for "PFMEA light". Those folks today are the process experts of tomorrow, and should all be in the Advanced Design departments of their respective companies in 5 years, if they are allowed to focus on the development of the tools.

Don't want to come off too strong here, but I really appreciated the "PPAP Lite" link, and have saved it for training purposes.

Insect - I have something almost identical that I put together a number of years ago (in fact, it looks almost identical...I "appropriated" it from on-line!)

And just to be clear...I am not teaching these guys anything...Upper Mgmt decided these are our experts...though none of them have ever had any PFMEA training.

They are taking our existing FMEAs and updating them...and in the process attempting to improve them and make them a tool that can be used. My only role is as an ancillary member of the team.
