What condition is your company in, if you score 3.0?

attached is the audit sheet we use here, It is a little more conveluded than most 5S sheets as we add Safety. Are machines are cicra 1970 and most have no guards to protect the workers or trays to collect the large amount of metal chips we produce.


On the form the auditor answers each questrion yes, no, or not applicable (one sheet covers all areas) Then they total the yes's and no's in each column. Then they add the total yes's and no's together (10 yes + 5 no's =15, NA's are not counted) Then divide the yes's by the total (10/15 = your score for that column ie.. 10/15 = .666) add the column scores for the total area score.
With this method if you answer yes to every question you will have a 1 for the total column score, times 5 columns, you get a total score of 5.

Like I said, I was not very crazy about this sheet.

My concern with it is Yes/No can be somewhat subjective. Do they get 'Yes' if they do it 50% of the time? 75% of the time? 100% of the time? Does the scoring for 'Yes' change from auditor to auditor?

One way to take away some of the subjectivity is to assess each criteria on a scale of 0-4. If everything is perfect, 4 out of 4. If one thing is wrong, 3 out of 4. If two things are wrong, 2 out of 4. You get the idea. Of course, some common sense is required. If you see three empty coffee cups on someone's desk, I'd group them as 1 item...not as 3.

Try doing a search here in the Cove for 5S forms...we've had this discussion before and I don't think we all need to attach our forms here again. Besides, this thread is under the category of 'Lean'. If we're going to resume the 5S discussion on tools and methodology, we should do so in a previously started thread.
