attached is the audit sheet we use here, It is a little more conveluded than most 5S sheets as we add Safety. Are machines are cicra 1970 and most have no guards to protect the workers or trays to collect the large amount of metal chips we produce.
On the form the auditor answers each questrion yes, no, or not applicable (one sheet covers all areas) Then they total the yes's and no's in each column. Then they add the total yes's and no's together (10 yes + 5 no's =15, NA's are not counted) Then divide the yes's by the total (10/15 = your score for that column ie.. 10/15 = .666) add the column scores for the total area score.
With this method if you answer yes to every question you will have a 1 for the total column score, times 5 columns, you get a total score of 5.
