What is an Ideal Control Chart for Hospital Surgery Infections?

I have data of infections occurred due to surgeries for 40 months.
I have variable sample size for each month as you can image.

Here is what my data looks like

Month no of surgeries no of infections
1 208 1
2 225 3
3 201 4
4 236 0

I am asked to create a control chart and to provide recommendations. Can anyone suggest which control chart is ideal in this case ? I am debating between using "U" chart and p chart ? Any help is really appreciated, i am a newbie to the control chart worl

The answer depends upon if more than one infection can result (and be recorded) against the same surgery. If there can be multiple infections per surgery, then you should use a "u" chart. If the "no. of infections" is indeed the "no. of surgeries resulting in one or more infections", then you should use a "p" chart.

I will assume that you don't have a given standard value (standard value for average or standard value for standard deviation):

It is a one sided control chart, as the lower limit will be set to zero (i.e. you will not have a minus infection rate)

the second thing which is to answer your question: I would suggest a U-control chart (Number of Non-conformities per Unit)

i have attached an excel sheet for illustration, but i don't its statistically valid to build it using only 4 samples, u need at least 20.

Note 1: the sheet is protected, and the password is 123
Note 2: the average and the standard deviation will be fixed after 20 readings and will not change
