1. If your QA inspector detects a defective, first job is to quarentine the lot and decide on the disposition action of the defective lot (Remember to quarentine the lots till despatch end). Then, as a quality engineer, check immediately whether the parts currently in production are having the similar defect. Then, decide on the corrective action along with Production and relevant teams

2. In our organisation, We raise an internal flash report and fwd it to production department. The report will be signed by the production incharge and he is responsible of closing the flash report with corrective actions. Once the actions are implemented, he returns back the flash report with QA, which inturn is maintained as a record

Thanks tbsiva. If I have to decide on the disposition on the lot, what should I refer on to make the decision? is the defective need to category in minor or major defect?

I think there is no "one proper method" in this. Any organization could decide on their own on how to deal with it.
But basic rule shall be
1) proper action to be taken to segregate the NG parts
2) proper disposition method to be decided by authorised person
3) corrective action and correction to be carried out in a timely manner. if you are a ts company, corrective action impact should be considered as well
4) if repair or rework needs to be done, make sure authorised person has reconfirm the conformity of the parts after rework/repair.
hope this will help

i think you should make the disposition decision based on customer requirement, a specification issued by customer, industrial standards, customer drawing, fit and function and etc. but main concern is the customer requirement
you may define any defects into group for your analysis purpose

As pointed out by Jerry, there is no specific method or procedure... Every organisation has unique procedure for treating non confirming parts.. Also, it depends on the severity of the defect.. If it can be reworked and bring back to the required quality level, u can do rework...of course with customers prior acceptance... If it cannot be reworked at all... still as a one time deviation, you can request the customer whether the deviation can be acceptable... Else, the final option is to scrap the parts

Thanks for the advices from all, basically I feel better to make a disposition on the defective part but there is a difficulty when I try to feedback the defective issue to production superior. They seem not to answer properly the CAPA which was issued to them. This is the stress from them.

when we issued CAPA, it is not just by paper..another way to solve this u can try to sit together with production people and explain to them about the defects, why it need to be reject and show them what is your customer requirement, procedure etc..try to guide them to practice the right path.
