Good question, with several answers. The need to document your system grows with the size of a company as well as with its complexity. Imagine a blacksmith at the beginning of the 20th century. He had his customer right beside him, telling him what he wanted. Today few people ever meet a (external) customer... Thus, the need to document how the customers requirements are translated into instructions for your staff.

Ok, You are in the right forum...Sift through what you find here. I had a quick look and came up with the following threads:

Advice on Structuring Documentation
4.2.3d What kind of media are you using for your controlled documents? Procedures?
Which ISO9001:2000 Document Control System software?

Have a look in ISO9001:2000, Clause 4.2.1 note 2: What you find there will allow you to tailor your system to your needs. A small company will need less documentation than a big one. Have look at:

ISO 9000 in Mom and Pop Shops - What are the simple concepts?
Keeping Procedures Simple
Am I doing to much work?
ISO 9001: Avoiding Over Documentation

Hope that helps for starters?
